Registered Nurse (RN) - Home Health
About The Position
The Home Health Registered Nurse provides skilled, compassionate care to patients in their homes, helping them recover, manage chronic conditions, and live safely and independently. The role focuses on preventing hospitalizations, empowering patients and families, and improving long-term outcomes through trusted relationships and coordinated care. The nurse partners with physicians and an interdisciplinary team to support safe recovery and long-term health. This includes conducting comprehensive assessments and OASIS documentation, creating and adjusting individualized plans of care, educating patients and caregivers, coordinating services, and preparing patients and families for discharge. The position is with Interim HealthCare in Columbia, South Carolina.
Requirements
- Graduate of an accredited Registered Nurse program.
- Active South Carolina RN license or Compact License.
- Minimum one year of nursing experience.
Nice To Haves
- Experience in Home Health, Medical Surgical, Critical Care, or Case Management.
- Familiarity with OASIS documentation.
Responsibilities
- Provide skilled, patient-centered care in patients’ homes.
- Partner with physicians and the interdisciplinary team to support safe recovery and long-term health.
- Perform comprehensive patient assessments, including accurate OASIS documentation.
- Develop, implement, and adjust individualized plans of care.
- Monitor patient condition and promptly communicate changes to physicians to help prevent complications.
- Educate patients and caregivers about medications, treatments, disease management, and home safety.
- Coordinate care with therapists, aides, and other providers to ensure seamless, compliant care.
- Prepare patients and families for safe, successful discharge.
- Document care accurately in the EMR.
