About The Position

The RN performs a comprehensive physical/clinical assessment of MI Choice (Medicaid) Waiver and other program participants to identify program eligibility and supportive service needs. In cooperation with the Supports Coordinator Social Worker (SW), this position is also responsible for the Nursing Facility Level of Care (NFLOC) determination, the development of participant care plans, arrangement for health and health-related services prescribed under the care plan, and ongoing monitoring and reassessment of participants. Has as his/her primary duty the performance of work requiring advanced knowledge in a field of science or learning customarily acquired by a prolonged course of specialized instruction. Requirements

  • Current license with the State of Michigan to practice as a Registered Nurse.
  • BSN or ADN required.
  • One year of related nursing experience required, preferably with the elderly or disabled, and/or home health care.
  • Valid CPR/AED certification.
  • Ability to communicate with elderly or disabled participants on a one-to-one basis.
  • Analytic and organizational abilities.
  • Interviewing and listening skills.
  • Assertiveness skills.
  • Ability to work effectively as a member of an interdisciplinary team.
  • Must be able to read and interpret documents such as Medicaid policies and interpret supporting documentation.
  • Working knowledge of public programs including local community health and social service network required.
  • Must be willing and able to travel within all counties of the region (and at times out of the region area) in a reliable automobile.
  • Computer experience necessary.

Nice To Haves

  • Experience in community health programs preferred.
  • Medication reconciliation skills preferred.
  • Additional related experience or combination of education and experience may be considered if, in the opinion of the organization, equivalency is established.

Responsibilities

  • Communicates with elderly or disabled participants on a one-to-one basis.
  • Reads and interprets documents such as Medicaid policies and interpret supporting documentation.
