Requirements

• Bachelor of Science degree in Nursing.
• License to practice as a Registered Nurse in the State of Michigan.
• Analytical skills necessary to evaluate patient status, health care procedures/techniques, and to monitor quality of patient care.
• Interpersonal skills necessary to work productively with all levels of hospital personnel.
• Verbal and written communication skills to communicate effectively with diverse populations including employees, patients and patient families.

Nice To Haves

• Six to twelve months of experience in ambulatory care or community health preferred.

Responsibilities

• Coordinates the implementation of the treatment plan identified by the health care provider in an ambulatory/outpatient setting.
• Performs assessment of clients to facilitate appropriate access and level of care required.
• Supports the designed process of care by triage, patient education, implementing consultations and referrals, participation in team conferences, discharge planning and telephone follow-up to clients.
