About The Position

National Jewish Health is seeking a Case Manager (Registered Nurse/RN) to support patients living with chronic conditions and empower them in their health journeys. This role is an essential link between clinical care and patient self-management, providing the guidance and coordination needed to improve outcomes and enhance quality of life. The Coordinated Care Management (CCM) Specialist plays a critical role in supporting patients with chronic conditions by providing education, care coordination, and ongoing case management. This position helps bridge the gap between clinical care and patient self-management by developing personalized care plans, conducting regular follow-ups, and assisting patients in navigating the healthcare system. Working collaboratively with providers and care teams, the CCM Specialist promotes improved health outcomes, medication adherence, and patient engagement.

Requirements

  • Graduation from an Accredited School of Nursing required.
  • Minimum 1 year of experience required.
  • Current Colorado Registered Nurse (RN) License and current CPR/BLS certification required upon hire for all areas.

  • Background in case management or chronic disease care preferred.

Responsibilities

  • Develops and updates individualized care plans tailored to patient needs and disease acuity.
  • Conducts health education focused on prevention, chronic disease management, and lifestyle modification.
  • Conducts regular patient follow-ups via phone using the ChronicCareIQ (CCIQ) platform.
  • Collaborates with providers and clinical teams to coordinate care and support patient engagement.
  • Promotes medication adherence, risk reduction, and improved health outcomes.
  • Assists patients with scheduling and navigating healthcare services while ensuring accurate documentation.
  • Maintains timely and accurate documentation in EHRs and other required platforms.
  • Ensures compliance with all applicable local, state, and federal care delivery regulations.
  • Demonstrates proficiency in utilizing the CCIQ platform to manage and coordinate patient care, ensuring accurate documentation, follow-up, and communication across the care team.
