About The Position

The Home Health Registered Nurse provides skilled nursing visits in Moberly, Missouri, and surrounding areas. The RN is a point of contact for patients and develops individualized plans of care according to physician orders, using physical assessment, diagnostic interpretation, and communication skills. The role involves working alongside nurses, physical therapists, speech therapists, and occupational therapists. Nurses complete charting during visits with support from Roger’s AI-powered system.

Requirements

Current Registered Nurse or Licensed Practical Nurse license in Missouri.
Valid driver’s license and a vehicle with current auto insurance.
Ability to pass a drug test, background check, and physical.

Nice To Haves

Home Health or Critical Care experience preferred.

Responsibilities

Assist in developing, reviewing, and revising the plan of care.
Provide nursing care in accordance with the physician’s plan of treatment and the individual plan of care, as authorized by the client’s payer source.
Observe and record the client’s reactions to treatment and report changes in condition to appropriate personnel and/or the physician.
Provide ongoing instruction and supervision to the client, family, or other health team personnel regarding the plan of treatment.
Participate in education programs, quality assessment activities, nurses’ meetings, and other staff meetings as required.
Monitor and record the patient’s condition and document provided care services through electronic charting.
