About The Position
Phoenix is hiring a Home Health Registered Nurse to provide skilled nursing visits in Columbia, Missouri, and surrounding areas. The nurse serves as a point of contact for patients and uses physical assessment, diagnostic interpretation, and communication skills to develop individualized plans of care according to physician orders and initial assessment findings. The role works alongside other nurses and physical, speech, and occupational therapists. Nurses complete charting during visits with support from Roger’s AI-powered tools. Full-time, part-time, and PRN arrangements are listed. A $5,000 sign-on bonus is offered, and the pay range is $70,000–$80,000. Requirements
- Current Registered Nurse or Licensed Practical Nurse license in Missouri.
- Driver’s license and a vehicle with current auto insurance.
- Ability to pass a drug test, background check, and physical.

- Home Health or critical care experience is preferred.
Responsibilities
- Assist in developing, reviewing, and revising the plan of care.
- Provide nursing care in accordance with the physician’s plan of treatment and the individual plan of care, as authorized by the client’s payer source.
- Observe and record the client’s reactions to treatment and report changes in condition to appropriate personnel and/or the physician.
- Provide ongoing instruction and supervision to the client, family, or other health team personnel regarding the plan of treatment.
- Participate in education programs, quality assessment activities, nurses’ meetings, and other staff meetings as required.
- Monitor and record the patient’s condition and document care services through electronic charting.
