RN Referral Coordinator
About The Position
BJC Home Care offers a comprehensive range of home care services, including skilled nursing, hospice, rehabilitation therapy, home infusion therapy, and more. The Central Referral Center (CRC) is the starting point for patient referrals, coordinating services for patients needing home care. The CRC includes nurses who handle physician office referrals and is staffed 24/7 to facilitate home care coordination for patients in emergency rooms and other situations. This role is responsible for processing referrals accurately and efficiently, ensuring timely transmission to operational areas, and serving as a liaison for all product lines to promote customer satisfaction.
Requirements
- Nursing Diploma/Associate's
- 2-5 years of experience
- RN license
- Must be able to work 1-2 days a week
- Ability to support IL team needs
- Coordinating with multiple hospital locations in IL
- Supporting both hospice and home health needs!
Nice To Haves
- Hospital experience is a plus!
- Bachelor's Degree in Nursing/Social Work/related
- 5-10 years of experience
Responsibilities
- Screens referrals to ensure patients meet agency admission criteria and comply with state and federal regulations.
- Verifies insurance benefits, obtains pre-authorizations as necessary, assists billing department, and interprets company services, admission criteria and medical policies to referral sources and individuals seeking information.
- Tracks current clients that are either hospitalized, placed in a short-term skilled facility or in a rehabilitation facility.
- Communicates and coordinates discharge planning and educates internal and external customers about Home Care Services and/or outpatient services.
