About The Position

The RN (Registered Nurse) Quality Assurance Coordinator is responsible for assisting in overall quality improvement activities for Nascentia Health CHHA and LHCSA divisions. This role involves reviewing clinical records, participating in chart audits, and analyzing documentation to identify trends. The coordinator will follow up with staff and management on identified issues, assist clinical leadership with OASIS education and documentation, and communicate with the Manager regarding client or staff concerns. Additionally, the position requires following up on documentation deficiencies, participating in staff training, performing assigned quality improvement activities, and communicating agency policies. The role promotes quality improvement in clinical staff activities, maintains knowledge of agency policies and quality improvement standards, and demonstrates knowledge of OASIS assessments and Medicare/Medicaid/private insurance. A working knowledge of coding principles for home care is also necessary. Other expectations include providing excellent customer service, maintaining patient confidentiality, fostering positive relationships, keeping the Manager informed of problems, and attending meetings as needed. All other duties as assigned will also be performed.

Requirements

  • Current New York State licensed Registered Nurse
  • Minimum 2 years working in home care.
  • Excellent organizational skills.
  • Good attention to detail and accuracy.
  • Strong critical thinking and assessment skills.
  • Ability to work independently and be self-directed.

Responsibilities

  • Reviews clinical records.
  • Participates in review of chart audits utilizing SHP reports (if applicable) and best practice following policies and procedures.
  • Completes all LHCSA audits.
  • Reviews client records, orders and other communication for quality and safety of care.
  • Analyzes documentation and reports trends to Manager.
  • Follows up with staff/management regarding issues identified in review process.
  • Assists clinical leadership in working with clinical staff on questions, OASIS education/clarification, documentation issues, and clinical care interventions.
  • Communicates with Manager regarding issues or concerns with clients and/or clinical staff.
  • Follows up with staff regarding documentation deficiencies and/or discrepancies with OASIS, coding and assessment.
  • Participates in formal and informal training opportunities for clinical staff.
  • Performs quality improvement activities as assigned by Manager of Quality Improvement.
  • Communicates agency policies to staff.
  • Promotes quality improvement as it relates to the activities of clinical staff in accordance with organizational policies and procedures.
  • Maintains knowledge of and adherence to agency policies, procedures, along with quality improvement standards.
  • Demonstrates knowledge and use of agency policies and procedures.
  • Demonstrates knowledge of quality improvement standards.
  • Demonstrates knowledge of OASIS assessments.
  • Has a working knowledge of Medicare/Medicaid and private insurance.
  • Ability to learn and apply coding principles to home care.
  • Provides excellent customer service.
  • Maintains patient confidentiality.
  • Maintains positive relationships with co-workers, administrative staff, clinical staff, physician offices and patients and families.
  • Keeps Manager of Quality Improvement informed of problems as they arise.
  • Attends meetings and conferences as needed.
  • Perform all other duties as assigned.
