About The Position

The Quality Assurance (QA) Coordinator is an integral part of the facility's overall functioning, monitoring and auditing standards of person-served care and the quality of the electronic health record. This role ensures quality improvement standards are current and compliant with the facility's level of care, encompassing staff development, infection control, medical records, and quality improvement. The QA Coordinator collaborates with the QA committee and program at the facility level and monitors overall compliance with these aspects as they relate to current licensing and accrediting policies and regulations. The Crestwood Center San Jose Psychiatric Health Facility (PHF) provides a spacious environment with various rooms and a courtyard, dedicated to restoring hope, self-empowerment, and recovery. It offers stabilization for acute psychiatric crises, including medication management, WRAP, DBT support, behavioral interventions, psychotherapy, and psychosocial education, with a focus on a thorough transition plan. Mental Health Rehabilitation Centers (MHRCs) provide psychosocial rehabilitation in secure, residential settings for brief lengths of stay.

Requirements

  • A degree in a health-related field and/or two years’ experience in health care education.
  • Experience working with health care agencies and/or regulatory agencies.
  • Knowledge of Title 9, 22, OSHA, HCAI, HIPAA, and CARF.
  • Ability to engage in performance improvement data collection, analysis, and implementation of QA plans.

Nice To Haves

  • A minimum of one year of experience in medical records and/or quality improvement is preferred.

Responsibilities

  • Monitor and audit the identified standards of person-served care.
  • Monitor and audit the quality of the entire electronic health record.
  • Ensure quality improvement standards are up to date and in compliance with the facility's level of care.
  • Collaborate with the QA committee and program at the facility level, encompassing aspects of staff development, infection control, medical records, and quality improvement.
  • Monitor overall compliance with staff development, infection control, medical records, and quality improvement as they relate to current licensing and accrediting policies and regulations.
