About The Position
Capital Health is seeking a Primary Care Nurse Triage Registered Nurse (RN) to provide telephonic and electronic clinical assessment, triage, and care coordination for patients within a primary care practice. This role is crucial for ensuring timely, safe, and appropriate clinical disposition based on standardized protocols and professional nursing judgment. The RN will collaborate closely with physicians, advanced practice providers (APPs), clinical staff, and support teams to optimize patient access, safety, and continuity of care.
Requirements
- Registered Nurse (RN) license
- Experience in telephonic and electronic clinical assessment, triage, and care coordination.
- Knowledge of standardized protocols and professional nursing judgment.
- Ability to collaborate effectively with physicians, APPs, clinical staff, and support teams.
