RN On Call Hospice
About The Position
Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services to help people receive care at home. The Hospice RN On-Call provides after-hours clinical support, triages patient needs, and coordinates care to support continuity and quality outcomes. The role includes remote and field-based nursing support, assessment of patient conditions, and collaboration with interdisciplinary teams to address urgent and emergent patient needs. The schedule begins Friday at 5:00 PM and ends Monday at 8:00 AM. Patient care territories include Luzerne, Wyoming, Lackawanna, Susquehanna, and Wayne counties. Requirements
- Current RN license specific to the state(s) assigned for work.
- Current CPR certification.
- At least 1 year of nursing experience; hospice, home health, or acute care experience is preferred.
- Valid driver's license, reliable transportation, and liability insurance.
- Pass a drug test before beginning employment.

Responsibilities
- Provide on-call nursing support, triage patient needs, and determine appropriate interventions.
- Assess patient conditions by phone and/or through in-person visits, and escalate care as needed.
- Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery.
- Provide patients and caregivers with education about symptom management and care plans.
- Maintain accurate and timely documentation of patient interactions and clinical decisions.
