About The Position

Tapestry Hospice & Palliative Care is a family-owned and operated hospice provider serving North Georgia. The On-Call RN provides holistic nursing care to terminally ill patients and their families after normal business hours, including appropriate support for families at the time of death. Care is based on the nursing process and includes patient assessments, identifying key problems, patient education, interventions, implementation, and evaluation of patient outcomes.

The RN creates, plans, and directs the hospice care team and works with the Interdisciplinary Team, patients, and loved ones while maintaining high standards of care and professional nursing clinical competency. The role includes performing admission evaluations and pronouncements and conducting follow-up visits. The schedule requires availability Friday through Sunday.

Requirements

• Certified Registered Nurse.
• Ability to work Friday through Sunday.
• Excellent clinical assessment skills.
• Ability to provide patient education and evaluate patient outcomes.
• Time management skills.
• Advanced knowledge of pain management and symptom control.

Nice To Haves

• Hospice case management experience.
• Critical care, surgery, or oncology experience.

Responsibilities

• Create, plan, and direct the hospice care team using nursing processes of assessment, interventions, implementation, and evaluation.
• Provide holistic nursing care to terminally ill patients and their families after normal business hours.
• Provide appropriate support to families at the time of death.
• Assess patients, identify key problems, provide patient education, and evaluate patient outcomes.
• Interact effectively with members of the Interdisciplinary Team, patients, and their loved ones.
• Perform admission evaluations and pronouncements.
• Conduct follow-up visits.
• Provide care while maintaining high standards of care and professional nursing clinical competency.
