About The Position

Angels of Care Pediatric Home Health is seeking compassionate Registered Nurses (RN) and Licensed Vocational Nurses (LVN) to provide direct, one-on-one medical care to medically fragile infants, children, and adolescents in their homes. The role involves focusing on the patient's medical needs and treatment according to the physician's plan of care, conducting ongoing assessments, administering medications and therapies, coordinating care, educating family members, and implementing preventative initiatives to ensure quality care. Requirements

Responsibilities

Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their home according to the active plan of care, as given by the physician and other appropriate personnel.
Conduct on-going patient care and assessments.
Administration of prescribed medication, treatments, and therapies.
Coordination of care
Educate family members on patient clinical care to enhance positive outcomes
