About The Position

The Clinical Nurse Leader works within Mozaic Senior Life’s household model of care. Together with a Household Coordinator and Lead Homemaker, the Clinical Nurse Leader helps create a home and community environment that supports residents’ quality of life, personal growth, and quality of care. Clinical Nurse Leaders and Household Coordinators form a Neighborhood Council that guides and supports the households under the guidance of the Vice President, Nursing and the Administrator.

The Clinical Nurse Leader performs nursing duties within the scope of practice defined by the State of Connecticut and the RN/LPN role. The position is responsible for the overall delivery of care and supports the household team in ensuring residents receive quality care and quality of life. The schedule is 37.5 hours per week on the 11 p.m.–7 a.m. shift, including every other weekend. Candidates must have at least one year of experience; skilled nursing facility or long-term care experience is preferred.

Requirements

• Graduate from an accredited school of nursing.
• At least one year of clinical experience; candidates with less than one year will not be considered.
• Current RN or LPN licensure in the State of Connecticut.
• Current BLS/CPR certification from the American Red Cross or American Heart Association.

Nice To Haves

• Experience in a skilled nursing facility or long-term care environment.
• At least one year of clinical experience in a long-term care setting.

Responsibilities

• Perform nursing duties within the scope of practice defined by the State of Connecticut and the RN/LPN role.
• Assume responsibility for the overall delivery of care to household residents.
• Support the household team in ensuring quality of care and quality of life for residents.
• Contribute to the household model of care alongside the Household Coordinator and Lead Homemaker.
• Participate in the Neighborhood Council’s guiding and supporting role for the households.
