About The Position

Beacon Hospice, part of Amedisys, is hiring an RN case manager to provide hospice care to patients at home. The role involves assessing patients, collaborating with the care team to develop and implement care plans, maintaining accurate patient records, managing and documenting pain and symptoms, and teaching patients, families, and caregivers about appropriate care techniques. The nurse also communicates regularly with the clinical manager and care team and provides on-call services as assigned. This is a part-time weekend position with an hourly pay range of $45.00 to $53.00. Compensation may vary by location, job-related knowledge, skills, and experience.

Requirements

• One year of nursing experience.
• Current RN license specific to the state(s) where assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to other hospice care disciplines as needed, or document the rationale for not doing so.
• Develop, prepare, and maintain individualized patient care progress records accurately, promptly, and according to care center policies.
• Record changes and outcomes related to pain and symptom management, as appropriate.
• Teach patients, families, and caregivers appropriate patient care techniques.
• Communicate regularly with the clinical manager and care team.
• Perform on-call responsibilities and provide on-call services to patients and families as assigned.
• Perform other duties as assigned.
