About The Position

Sage Group HealthCare Staffing is seeking a Hospice & Home Health RN for a contract position in Barre, VT. This role involves providing RN Case Management for Home Hospice patients, with the flexibility to also manage Home Health patients. The RN Case Manager will screen, evaluate, re-evaluate, and treat patients, while coordinating and communicating treatment activities with the interdisciplinary team, patient, and caregivers. A key responsibility includes developing and periodically reviewing a care plan in collaboration with the patient's physician, patient, and family. The role requires ongoing assessment of the patient's clinical, functional, and emotional status, communicating significant changes to the physician and colleagues, and teaching patients and caregivers self-care techniques. Medication, diet, and other physician-ordered instructions will be provided, along with opportunities for health counseling.

Requirements

  • Registered Nurse: 2 years - Required
  • Hospice in home setting RN experience: 2 years – Required
  • Home Health RN experience - Required
  • OASIS experience - Required
  • Must have valid Driver’s License and Personal Auto Insurance
  • Must be willing to case manage home health patients when needed

Nice To Haves

  • HCHB EMR experience - Preferred
  • BSN or an equivalent – Highly preferred

Responsibilities

  • Provide RN Case Management for Home Hospice patients.
  • Willing to Case Manage Home Health patients when needed.
  • Screen, evaluate, re-evaluate, and treat patients.
  • Coordinate and communicate treatment activities with the interdisciplinary team members and the patient.
  • Develop a care plan in collaboration with the patient's physician, the patient, and their family, or the primary caregivers upon admission.
  • Periodically review the patient care plan and recertify the patient care plan as appropriate.
  • Coordinate the patient's care plan through interdisciplinary communication and case conferences.
  • Advise other disciplines of discharges in a timely manner.
  • Provide ongoing assessment of the patient's clinical, functional, and emotional status to identify problems and/or progress toward desired goals.
  • Communicate significant changes to the patient's physician and colleagues.
  • Document evaluations, treatment goals, and objectives.
  • Teach the patient and caregiver self-care techniques as appropriate.
  • Provide medication, diet, and other instructions as ordered by the physician.
  • Utilize opportunities for health counseling with patients and caregivers.
