About The Position
As an RN Case Manager, you’ll coordinate compassionate, individualized hospice care by assessing patients, partnering with the interdisciplinary care team, and developing plans that support each patient’s needs. The role is with Amedisys, part of the Optum family of businesses, which provides home health, hospice, and community-based services.
Requirements
- Current RN license specific to the state(s) assigned.
- Valid driver's license, reliable transportation, and liability insurance.
- Current CPR certification.
- At least 1 year of nursing experience.
Responsibilities
- Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
- Make referrals to other hospice care disciplines as needed, or document the rationale for not doing so.
- Develop, prepare, and maintain individualized patient care progress records accurately, promptly, and according to care center policies.
- Record changes and outcomes related to pain and symptom management as appropriate.
- Teach patients, families, and caregivers appropriate patient care techniques.
- Regularly communicate with the clinical manager and care team.
- Perform on-call responsibilities and provide on-call services to patients and families as assigned.
- Perform other duties as assigned.
