About The Position

Amedisys is hiring a full-time, day-shift RN Case Manager for hospice care. The role supports patients at home by assessing their needs and working with the care team to develop and implement individualized plans of care. The listed annual salary range is $100,000–$105,000, and the posting offers a $7,500 sign-on bonus. The RN Case Manager maintains accurate and timely patient progress records, monitors and records pain and symptom management outcomes, and teaches patients, families, and caregivers appropriate care techniques. The position also includes coordinating referrals to other hospice disciplines, communicating regularly with the clinical manager and care team, and providing on-call services as assigned. Requirements

One year of nursing experience.
Current RN license specific to the state or states where assigned to work.
Current CPR certification.
Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
Make referrals to other hospice care disciplines as needed, or document the rationale for not doing so.
Develop, prepare, and maintain individualized patient care progress records accurately and promptly, in accordance with care center policies.
Record pain and symptom management changes and outcomes as appropriate.
Teach patients, families, and caregivers appropriate patient care techniques.
Communicate regularly with the clinical manager and care team.
Perform on-call responsibilities and provide on-call services to patients and families as assigned.
Perform other duties as assigned.
