About The Position

Amedisys is hiring a full-time, day-shift RN Case Manager Hospice to provide hospice care to patients at home. The role involves assessing patients, collaborating with the care team to develop and implement individualized care plans, documenting progress and symptom-management outcomes, and educating patients, families, and caregivers. The case manager also communicates regularly with the clinical manager and care team and performs assigned on-call duties. The listed annual salary range is $85,000–$90,000, with compensation varying by geographic market, job-related knowledge, skills, and experience. Benefit eligibility may vary by position and shift status; participation in some benefits requires enrollment in an Amedisys medical plan.

Requirements

• One year of nursing experience.
• Current RN license for the state or states where assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to other hospice care disciplines as needed, or document the rationale for not doing so.
• Develop, prepare, and maintain individualized patient care progress records accurately and promptly, in accordance with care center policies.
• Record changes and outcomes related to pain and symptom management as appropriate.
• Teach patients, families, and caregivers appropriate patient care techniques.
• Communicate regularly with the clinical manager and care team.
• Perform assigned on-call responsibilities and provide on-call services to patients and families.
• Perform other duties as assigned.
