About The Position

Trustbridge Hospice, a member of Empath Health, is seeking a caring and motivated Hospice Registered Nurse (RN) Case Manager to provide patient-centered hospice care in the northeast Broward County area. The position involves advocating for patients and families, coordinating care with physicians and the interdisciplinary team, and helping families through the hospice journey. The schedule is Monday through Friday, 8:30 a.m. to 5:00 p.m., with a weekend requirement. Trustbridge Hospice serves families across Palm Beach and Broward counties with community-based care focused on comfort, dignity, and peace for people facing advanced illness.

Requirements

Currently licensed as a Registered Nurse in the State of Florida.
Current Basic Life Support (BLS) certification.
Understanding of and ability to apply hospice nursing characteristics, including case management, crisis intervention, communication, teaching, thanatology skills, and needs.
Basic computer skills and experience with EMR software are a plus.
Adequate and reliable transportation with adequate insurance and a valid Florida driver’s license.

Nice To Haves

Bachelor of Science in Nursing (BSN).
Hospice certification.
One year of experience in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing, or hospice nursing.
Basic computer skills and EMR software experience.

Responsibilities

Serve as an advocate for patients and families, ensuring their needs are recognized and addressed.
Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team.
Develop and implement individualized plans of care as part of the Interdisciplinary Group (IDG).
Monitor patient conditions, provide interventions, and deliver education to enhance comfort and quality of life.
Address concerns promptly and thoughtfully to support satisfaction and trust.
