About The Position
Dignity Health at Home seeks a Registered Nurse to provide personalized nursing care in patients’ homes. The role offers a 1:1 patient-clinician ratio and time to educate, listen to, and support patients throughout their healing process. The nurse conducts care from the initial visit through recovery, adapting plans as patients’ needs and progress change. The position also provides opportunities for professional development and potential advancement into clinical leadership, operations, or education. AI-powered clinical documentation is used to reduce documentation burden and support more patient interaction. The Home Health RN develops and adjusts comprehensive care plans, coordinates with physicians, therapists, and support staff, and provides teaching and emotional support to patients and families. The nurse oversees a caseload and guides LPNs, LVNs, and Home Health Aides, while using technology and best practices to support care delivery and accurate documentation.
Requirements
- Graduate of an accredited nursing program.
- Active RN license in the state served.
- At least 1 year of Home Health experience.
- Exceptional critical-thinking skills and proven clinical expertise.
- Strong case-management abilities and commitment to patient-centered care.
- Current CPR/BLS certification.
- Valid driver’s license and reliable transportation.
Responsibilities

- Collaborate with physicians, therapists, and support staff to coordinate holistic care.
- Educate and provide emotional support to patients and their families.

- Use technology and best practices to optimize care delivery and maintain accurate documentation.
- Support patients through the healing process, from the initial visit through recovery.
