About The Position

CommonSpirit Health at Home provides home health nursing focused on meaningful patient relationships, individualized care, and dedicated time. The RN delivers personalized care in patients’ homes, guides their recovery, and supports patient independence and well-being. The role is offered full-time or part-time on the day shift, 8:00 AM to 5:00 PM, and has no after-hours on-call requirements. The position uses NestMed ambient AI technology and offers opportunities for professional development and advancement into clinical leadership, operations, or educational initiatives.

Requirements

• Graduate of an accredited nursing program.
• Active RN license in the state served.
• At least 1 year of nursing experience.
• Exceptional critical-thinking skills and demonstrated clinical expertise.
• Strong case-management abilities and commitment to patient-centered care.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

Nice To Haves

• Home health experience is a significant plus.

Responsibilities

• Deliver high-quality, personalized nursing care in patients’ homes.
• Develop, implement, and adjust comprehensive individualized care plans to promote patient independence and well-being.
• Coordinate care with physicians, therapists, and support staff as part of a multidisciplinary team.
• Educate and support patients and their families, including providing emotional support.
• Oversee a patient caseload and guide LPNs, LVNs, and Home Health Aides.
• Use technology and best practices to optimize care delivery and maintain accurate documentation.
