About The Position

Corewell Health is hiring a Registered Nurse for its Visiting Nurses Association Home Care Skilled Nursing Team, serving the North Holland area. The RN works under the direct supervision of the Nursing Manager and in collaboration with an interdisciplinary team, applying clinical judgment to assess patients and implement treatment plans. The role involves autonomous home visits, providing care in patients’ homes and supporting patients’ ability to manage their health. Personalized assignments are close to home, with nurses typically visiting 5–7 patients per shift. The service area spans West-Central Michigan. The position is full-time, day shift, with 40 scheduled hours per week. The stated hours are 8:00 a.m. to 4:30 p.m., Monday through Friday, with every fourth weekend. The broader home-care schedule description also mentions working every third holiday and being on call 1–2 days in a six-week rotation. Eligible licensed RNs who are new to an RN role at Corewell Health may qualify for sign-on bonus incentives of up to $10,000. Requirements

Associate’s degree in Nursing.
Michigan Registered Nurse (RN) license, required upon hire.
Michigan driver’s license, required upon hire.
Basic Life Support (BLS) certification from the American Heart Association or American Red Cross within 90 days for VNA and Home Care business lines.

Nice To Haves

Bachelor’s degree in Nursing.
One year of relevant acute care, emergency, or rehabilitation facility experience for Home Care business lines.

Responsibilities

Assess, monitor, and detect changes in patients’ conditions, and take appropriate action to prevent physiological complications associated with health problems or medical treatment.
Communicate effectively with staff, patients, residents, and families.
Develop and maintain an individualized patient plan of care in collaboration with the patient.
Coordinate with other members of the health care team to support continuity of care and coordination of services.
Act as a patient and family advocate.
Evaluate patients’ and significant others’ educational needs and develop and deliver appropriate teaching strategies or make referrals.
Observe and report patients’ conditions, needs, and intervention outcomes to physicians and other team members.
Identify problems and emergency situations and initiate immediate life-saving measures in the absence of a physician.
Implement the plan of care by providing nursing services, including teaching, treatments, diagnostics, and preventive procedures.
Regularly evaluate patient progress toward established goals, recommend revisions to the plan as needed, and plan for termination of service or discharge.
Assume a leadership role and delegate appropriately to support staff to facilitate safe, quality, cost-effective patient care.
Participate in ongoing process-improvement activities, including clinical paths and quality-assurance processes.
Monitor processes and services for quality, cost-effectiveness, and efficiency.
As specified by the business entity, perform medication administration according to regulatory guidelines.
As specified by the business entity, inform the MDS coordinator of significant patient changes relevant to MDS completion.
As specified by the business entity, teach and supervise care provided by CNAs, LPNs, or other nursing personnel in accordance with regulatory and agency guidelines.
