About The Position

BrightStar Care of Chicago provides skilled and non-skilled home care services to clients across the Chicago area. We're committed to delivering higher standards of care through joint-commission accredited practices and a strong, supportive clinical team. We're looking for an experienced RN Case Manager to join our clinical team. This role is primarily field-based, conducting patient assessments and re-assessments in clients' homes throughout Chicago, while managing an active caseload and serving as a clinical resource for our field nursing staff. The ideal candidate combines strong critical care clinical judgment with the flexibility and independence needed to manage a mobile, client-facing schedule.

Requirements

  • Active, unrestricted RN license in the state of Illinois
  • Critical care clinical experience
  • Reliable personal vehicle, valid driver's license, and current auto insurance
  • Comfortable with extensive daily driving across the greater Chicago area
  • Negative TB Test and Physical
  • Current CPR Certification
  • Strong assessment, critical thinking, and clinical decision-making skills
  • Excellent communication skills — comfortable being a clinical resource for peers
  • Proficiency with electronic documentation systems

Nice To Haves

  • Minimum 1 year of ICU experience
  • Home care, hospice, or case management experience

Responsibilities

  • Conduct comprehensive new patient (start of care) assessments in clients' homes
  • Perform scheduled and as-needed re-assessments to evaluate changes in patient condition and care needs
  • Manage an active caseload, coordinating care plans, updating documentation, and ensuring continuity of care
  • Serve as a clinical point of contact for field nurses, answering questions and providing guidance on complex cases
  • Communicate with physicians, families, and interdisciplinary team members regarding patient status and care plan changes
  • Ensure care plans and documentation meet regulatory and Joint Commission compliance standards
  • Identify and escalate clinical concerns or changes in condition appropriately
  • Participate in office-based team meetings, case conferences, and administrative work (~1 day/week equivalent)
