RN Field Case Manager- Palos Park, Hickory Hills, Crestwood & Surrounding Areas
About The Position
The Field Registered Nurse (RN) Case Manager is a member of the interdisciplinary team and provides skilled nursing assessment and hospice care to maximize the comfort and health of patients and their families or caregivers, consistent with Lightways Hospice and Serious Illness Care policies and procedures. The RN plans and directs services to meet the needs of individuals and families or caregivers in their homes and communities. The position covers Southern Cook County, including Palos Park, Crestwood, Hickory Hills, and surrounding areas. The schedule is Monday through Friday, with a minimum of 20 on-call hours per month (which may increase) supporting the entire service area.
Requirements
- Current RN license.
- Graduate of an accredited nursing program.
- Minimum of 2 years of nursing experience.
- Valid driver’s license and insured, reliable vehicle.
Nice To Haves
- At least 1 year of experience in public health, home care, or hospice is preferred.
Responsibilities
- Deliver skilled nursing care and hospice services in home and community settings.
- Conduct comprehensive patient assessments and develop individualized care plans.
- Administer medications and treatments while monitoring patient symptoms and outcomes.
- Educate patients and caregivers on care, self-management, and disease progression.
- Collaborate with the interdisciplinary team to ensure coordinated, high-quality care.
- Maintain accurate documentation and participate in on-call coverage as needed.
