About The Position

We help patients and their families experience a dignified, peaceful end of life. If you have both the heart and skills to work with hospice patients and families, consider joining our team. Location : Merrimack, NH Shifts: 36 - 4o hours per week, sharing in every other weekend and holiday rotation

Requirements

  • Graduate of an accredited school of nursing.
  • Associate's degree in nursing.
  • Strong skills in symptom management.
  • One year recent acute care experience.
  • Experience managing oncology patients and those with co-morbidities.
  • Registered as a RN with New Hampshire’s Board of Nursing.
  • CPR Certification.
  • Satisfactory criminal record.
  • Position requires sharing in every other weekend rotation and agency holidays.

Nice To Haves

  • Bachelor’s degree in nursing.
  • Community Health/Education experience.
  • Certification in hospice and palliative care a plus.
  • Knowledge or experience with end-of-life care and hospice care is beneficial.
  • EMR software proficiency. We use EPIC.

Responsibilities

  • Accessing, collecting data, and evaluating the patient's physical, spiritual, and emotional needs.
  • Provides preventative and therapeutic treatment under medical direction.
  • Creates a nursing plan of care with input from the patient, family, and provider to include measurable goals and behavioral objectives.
