About The Position

As an RN in our Hospice House/Inpatient Unit, you'll provide compassionate, patient-centered care to individuals with life-limiting illnesses while supporting their families through every stage of the hospice journey. Working alongside an interdisciplinary team, you'll deliver high-quality clinical care in a collaborative and supportive environment.

Requirements

Current Registered Nurse (RN) license in good standing with no disciplinary actions.
Current CPR certification.
Strong clinical assessment, symptom management, and care planning skills.
Experience documenting patient care in an electronic medical record system.
Valid Ohio driver's license, reliable transportation, and proof of auto insurance.
Ability to successfully complete a criminal background check and 10-panel drug screening.

Nice To Haves

Certified Hospice and Palliative Nurse (CHPN) certification preferred. Support is available for eligible employees pursuing certification.

Responsibilities

Deliver skilled nursing care using the nursing process, including assessment, planning, implementation, and evaluation.
Assess patients' physical, emotional, psychosocial, and spiritual needs and incorporate findings into individualized care plans.
Develop, implement, and regularly update patient-centered plans of care in collaboration with patients, families, and the care team.
Monitor changes in patient condition and communicate updates to the physician to ensure appropriate levels of care.
Review and adjust interventions, medications, and visit frequency as patient needs evolve.
Participate in interdisciplinary team meetings, care conferences, and admissions to coordinate comprehensive patient care.
Accurately document assessments, interventions, physician orders, and patient status in the electronic medical record.
Communicate patient updates through effective handoff methods, including SBAR, to support continuity of care.
Provide guidance and oversight to personal care specialists, volunteers, and other caregivers to ensure care aligns with the established plan.
Assist other departments and team members as needed to support exceptional patient care.
Participate in quality improvement initiatives that enhance patient outcomes and organizational excellence.
Serve as a preceptor for new team members when assigned.
