About The Position

We are looking for nurses to join our team! This role involves performing initial and ongoing patient/family assessments, providing palliative nursing care and symptom control, and developing interdisciplinary care plans. The nurse will also provide anticipatory guidance to patients and families and document all assessments and interventions in the EMR before the end of the shift. Effective communication in English, both verbally and in writing, is essential. The role also requires revising nursing diagnoses, goals, and interventions based on ongoing assessment and patient/family input.

Requirements

  • Graduate of an accredited nursing school.
  • Valid New York State nursing license.
  • Minimum of at least 2 years of nursing experience required.
  • Minimum of at least 1 year ICU, Med-Surg, ER or similar required.
  • Ability to exercise critical thinking skills.
  • Skill to prioritize and handle multiple tasks effectively.
  • Ability to lift at least 30 pounds of supplies and/or equipment.

Nice To Haves

  • One year of hospice experience preferred.
  • Venipuncture and IV placement certification preferred.
  • Bilingual is a plus!

Responsibilities

  • Perform initial and ongoing patient/family, physical, emotional, psycho-social, cultural and spiritual assessments to ensure and coordinate the delivery of comprehensive, holistic services by the Interdisciplinary Hospice Team.
  • Provide palliative nursing care and symptom control in accordance with physician orders and IDG plan of care.
  • Document and enter into EMR all assessments and interventions before the end of the shift.
  • Develop and prioritize the interdisciplinary care plan goals of the patient/family, hospice team and attending physician, prior to team meetings.
  • Provide anticipatory guidance to the patient/family to assist in preparing for coping with events as they occur.
  • Effectively communicate in English, both verbally (in person and by phone) and in writing.
  • Revise nursing diagnosis, goals and interventions, expected outcomes, and plan of care based on assessment of data and input of patients and family members prior to the weekly IDG meeting.
