RN Clinical Supervisor

Compassionate Care Home Health Agency•Visalia, CA

About The Position

The RN Clinical Supervisor serves as case manager for every patient seen by the nursing staff they supervise and provides ongoing audit and oversight from admission through discharge. The supervisor owns each assigned patient’s plan of care after it is established, coordinates care across disciplines, approves clinically appropriate changes, and ensures patients progress toward discharge goals and are discharged when goals are met or skilled need ends. The role provides clinical oversight of supervised caseloads, including chart reviews, care-coordination documentation, interdisciplinary communication, LVN supervisory visits, competency validation, and timely start-of-care and resumption-of-care activity. The supervisor conducts pop-up and joint visits, participates in QAPI, and may carry a limited patient caseload when staffing requires. The supervisor maintains current agency tracking tools and monitors past-due documentation and QA items. The role also supports staffing, new-staff orientation and training, and consistent accountability across disciplines and employment types. The supervisor is available during regular business hours, participates in the on-call rotation as directed, attends required meetings, and provides weekly status reports to the DPCS. The position requires driving and travel between patient homes, facilities, or other care locations as needed. Compensation is $125,000 per year.

  • Follow and enforce agency policies and procedures; remain available during regular business hours and participate in the on-call rotation as directed by the DPCS.
  • Attend required leadership, clinical, and case-conference meetings.
  • Submit weekly status reports to the DPCS covering caseload and census, staffing gaps, pending approvals, patients not progressing, past-due items, QA box status, and staff accountability actions.
  • Monitor staffing needs, identify gaps, and alert the DPCS and Scheduling before gaps cause missed visits or delayed SOCs.
  • Assist with new-staff orientation, field preceptorship, and competency validation, including training to the SURCH standard.
  • Maintain quality standards across disciplines and employment types, including full-time, part-time, per diem, and contracted staff.
  • Keep HHSM tracking tools current daily, including Prior Approval Form decisions, Discharge-Stabilization Tracking, the SOC Tracker for new frequencies, and the 5 Elements report.

  • Model compliance, documentation quality, timeliness, and work ethic; maintain the supervisor’s own documentation and tracking tools.
  • Initiate documented coaching and progressive corrective action; coordinate formal discipline with HR and handle contracted clinicians under their agreements.

  • Immediately escalate patient-safety, abuse, or neglect concerns.
  • Meet performance measures for approvals, documentation, tracking, reporting, meetings, on-call coverage, clinician audit compliance, and new-staff training.
