About The Position

Life’s At Home Care is seeking a Virginia-licensed RN to lead clinical care in a home care setting. The role combines independent field visits with collaborative clinical leadership and supports the Chesapeake branch. The RN Clinical Manager, also referred to as Director of Nursing, completes initial client assessments and required reassessments, develops and updates individualized care plans, monitors clinical risks, coaches caregivers, and partners with branch leadership to strengthen quality, safe client starts, and continuity of care. The position involves regular travel to clients’ homes and collaboration with office leadership. Routine after-hours on-call coverage is handled by a separate team member. The RN may occasionally be contacted to provide clinical guidance or escalation support for urgent clinical concerns outside normal business hours; these situations are expected to be infrequent. The RN is not responsible for routine scheduling or staffing calls. When assigned, the RN also assists with admission paperwork in addition to clinical assessments and care plans.
