RN Case Manager
About The Position
The RN Case Manager performs initial and ongoing comprehensive assessments of how a patient’s terminal disease affects physical, psychosocial, and environmental needs and activities of daily living. The role implements individualized plans of care, recommends revisions, and coordinates care among patients, caregivers, physicians, and other healthcare professionals. The RN Case Manager participates in interdisciplinary care planning, provides on-call service as assigned, submits documentation on time, and helps ensure compliance with applicable laws, Medicare regulations, and personnel policies and procedures.
Requirements
- Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
- Current State Registered Nurse license.
- Current driver’s license, reliable transportation, and ongoing valid and current auto liability insurance.
- Two years of RN experience in a healthcare or clinical setting preferred.

Responsibilities
- Conduct initial and ongoing comprehensive assessments of the impact of terminal disease on patients’ physical, psychosocial, and environmental needs and activities of daily living, including grief risk and cultural, spiritual, verbal, and non-verbal needs.
- Implement individualized plans of care and recommend revisions as necessary.
- Collaborate on care with patients, caregivers, physicians, and healthcare professionals.
- Attend Inter-Disciplinary Group (IDG) meetings; coordinate and participate in patient care planning and prioritization of needs; collaborate with the IDG to promote coordinated patient care.
- Perform assigned on-call responsibilities and provide on-call service to patients and families.
- Submit all documentation in a timely manner.
- Perform other duties as required.
- Ensure overall compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.
