About The Position

Heart to Heart Hospice provides compassionate care to patients with life-limiting illnesses and their loved ones. The RN Case Manager conducts comprehensive assessments, implements and updates individualized plans of care, coordinates care with patients, caregivers, physicians, and other healthcare professionals, and participates in interdisciplinary care planning. The role also includes on-call responsibilities, timely documentation, and ensuring compliance with applicable laws, regulations, and agency policies.

Requirements

Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
Current state Registered Nurse license.
Current driver’s license, reliable transportation, and ongoing valid and current auto liability insurance.

Nice To Haves

Two years of RN experience in a healthcare or clinical setting is preferred.

Responsibilities

Conduct initial and ongoing comprehensive assessments of how the terminal disease affects patients’ physical, psychosocial, and environmental needs and activities of daily living, including grief risk and cultural, spiritual, verbal, and non-verbal needs.
Implement each individualized plan of care and recommend revisions as necessary.
Collaborate on care with patients and caregivers, physicians, and healthcare professionals.
Attend Inter-Disciplinary Group meetings; coordinate and participate in patient care planning, prioritize needs, and collaborate with the group to promote coordinated care.
Perform assigned on-call responsibilities and provide on-call service to patients and families.
Submit all documentation in a timely manner.
Perform other duties as required.
Ensure compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.
