About The Position
Reliant at Home is seeking a PRN RN Case Manager to cover Gun Barrel City, Texas, and surrounding areas. Home Health experience is not required; knowledge of Homecare Homebase is a plus. The role involves providing home health nursing care, assessing patients during visits, administering prescribed medications, managing wounds, educating patients and families, and coordinating care with other professionals. The position also supports patient comfort through palliative care as needed and recommends devices or tools that may improve quality of life. Responsibilities
- Assess the patient's condition during every visit and chart observations.
- Perform evaluation tasks, including taking vital signs and reviewing medications.
- Administer medication as prescribed by the patient's physician.
- Dress or redress open wounds and assess healing progress.
- Educate patients and their families on home health care strategies and procedures.
- Coordinate with occupational therapists, physical therapists, physicians, and others involved in the patient's care plan.
- Provide palliative care as needed to keep the patient comfortable.
- Recommend devices or tools that may improve the patient's quality of life.
- Listen to patients and respond to their concerns or requests.
