About The Position

Elite Home Health & Hospice is seeking a dedicated Registered Nurse (RN) Case Manager to provide compassionate, patient-centered home health and hospice care in Moscow, Idaho. The role coordinates and delivers high-quality services that help patients live safely and comfortably at home, while building meaningful relationships with patients and their families. The RN Case Manager completes patient assessments, develops and adjusts individualized care plans, provides skilled nursing care, educates patients and families, and coordinates with the interdisciplinary care team. The position is full-time and offers pay of $41–$46 per hour, a comprehensive benefits package, mileage reimbursement, flexible scheduling, field autonomy, and opportunities for professional growth and development.

Requirements

Current Idaho RN license in good standing.
Graduate of an accredited nursing program.
At least one year of nursing experience.
Strong assessment, communication, and organizational skills.
Ability to work independently and effectively manage a caseload.
Current driver's license, reliable transportation, and auto insurance.

Responsibilities

Complete comprehensive patient assessments and develop individualized care plans.
Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
Provide skilled nursing care in accordance with physician orders and agency policies.
Monitor patient progress and modify care plans as needed.
Educate patients and family members regarding disease management, medications, and the plan of care.
Maintain accurate and timely clinical documentation.
Participate in interdisciplinary team meetings and case conferences.
Ensure compliance with federal, state, and agency regulations.
