About The Position

Elite Home Health & Hospice is seeking a Registered Nurse (RN) Case Manager in Moscow, Idaho. The role coordinates and delivers patient-centered home health and hospice services, helping patients live safely and comfortably at home while building relationships with patients and families. Requirements

Current Idaho RN license in good standing.
Graduate of an accredited nursing program.
At least one year of nursing experience.
Strong assessment, communication, and organizational skills.
Ability to work independently and effectively manage a caseload.
Current driver's license, reliable transportation, and auto insurance.

Nice To Haves

Home health or hospice experience preferred.

Responsibilities

Complete comprehensive patient assessments and develop individualized care plans.
Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
Provide skilled nursing care in accordance with physician orders and agency policies.
Monitor patient progress and modify care plans as needed.
Educate patients and family members about disease management, medications, and the plan of care.
Maintain accurate and timely clinical documentation.
Participate in interdisciplinary team meetings and case conferences.
Ensure compliance with federal, state, and agency regulations.
