About The Position
As a Floating RN Case Manager, your day is spent on the road, in different communities, meeting new patients, and supporting different care teams. You still get the best part of home health, 1:1 patient care, but without the monotony of one fixed territory. This role is for nurses who want flexibility, independence, and a change of pace. It offers variety without losing structure, and the opportunity to step into new situations and figure things out quickly.
Requirements
- Active Minnesota Registered Nurse (RN) license.
- Comfortable working independently in a mobile, field-based role.
- Strong clinical judgment and adaptability.
- Reliable transportation and willingness to travel.
Nice To Haves
- Home health experience preferred, but not required.
Responsibilities
- Travel across the metro to support different territories and care teams.
- Provide skilled nursing visits, assessments, and care planning.
- Complete OASIS visits and coordinate patient care.
- Build trust quickly with patients and families.
- Help maintain consistent care during growth or staffing changes.
