You will not be tied to one unit or building but will move where the need is to support teams and make an immediate impact. It provides higher earning potential with a base salary plus incentive visits, a floating territory model for variety, and 1:1 patient care in homes and community settings. You will have autonomy in the field with strong clinical leadership support, backed by a 30+ year Minnesota-based home care leader.

Requirements

• Active Minnesota Registered Nurse (RN) license.
• Strong clinical judgment and ability to adapt quickly.
• Comfortable working independently in a field-based, travel-focused role.
• Reliable transportation and willingness to drive throughout the metro.

Nice To Haves

• Experience in home health preferred, but we will train the right RN.

Responsibilities

• Step into different territories to support caseload coverage and census needs.
• Provide skilled nursing care, assessments, and care planning.
• Complete OASIS visits and coordinate interdisciplinary care.
• Educate patients and families on care plans and recovery goals.
• Collaborate with physicians, therapists, and care teams.
• Travel throughout the metro while maintaining flexibility and independence.
