About The Position

As an RN Case Manager, you will not be tied to a single unit or building but will instead move where the need is, supporting teams and making an immediate impact. Requirements

• Active Minnesota Registered Nurse (RN) license.
• Strong clinical judgment and ability to adapt quickly.
• Comfortable working independently in a field-based, travel-focused role.
• Reliable transportation and willingness to drive throughout the metro.

• Experience in home health preferred.

Responsibilities

• Step into different territories to support caseload coverage and census needs.
• Provide skilled nursing care, assessments, and care planning.
• Complete OASIS visits and coordinate interdisciplinary care.
• Educate patients and families on care plans and recovery goals.
• Collaborate with physicians, therapists, and care teams.
• Travel throughout the metro while maintaining flexibility and independence.
