RN Case Manager – Flexible Territory Float, Home Health
About The Position
This role is designed for nurses who want flexibility, independence, and a change of pace. As a Floating RN Case Manager, your day is spent on the road, in different communities, meeting new patients, and supporting different care teams. You will provide 1:1 patient care without the monotony of one fixed territory. This position offers variety without losing structure, and is ideal for those who prefer 1:1 care over high patient ratios and enjoy stepping into new situations and figuring things out quickly. Requirements
- Active Minnesota Registered Nurse (RN) license.
- Comfortable working independently in a mobile, field-based role.
- Strong clinical judgment and adaptability.
- Reliable transportation and willingness to travel.
Nice To Haves
- Home health experience preferred, but not required.
Responsibilities
- Travel across the metro to support different territories and care teams.
- Provide skilled nursing visits, assessments, and care planning.
- Complete OASIS visits and coordinate patient care.
- Build trust quickly with patients and families.
- Help maintain consistent care during growth or staffing changes.
