About The Position

The RN Case Manager coordinates longitudinal care for patients with complex medical, social, and psychological needs. The role serves as the primary liaison among patients, medical providers, and the insurance and benefit system. Its goals are to promote optimal patient outcomes, prevent unnecessary hospital readmissions, ensure effective use of resources, and advocate for patients’ health goals.

The schedule is 8:00 a.m.–4:30 p.m., with shifts Monday through Sunday. PRN staff fill in throughout the week, including weekends. Orientation lasts 12 weeks; the first 5–6 weeks are Monday through Friday, 8:00 a.m.–4:30 p.m. The position is based at the Medical Center in Fort Worth, in the Case Management department, and Cook Children’s is a Texas-only employer. Employees must maintain their primary residence in Texas and maintain the licenses and credentials required for the position.

Requirements

• Three years of experience in one or a combination of specialty nursing, case management, care coordination, utilization review, patient intake, discharge planning and troubleshooting fund resources, quality assurance, clinical pathways, continuous quality improvement, or state and federal health plans or commercial insurance plans in a clinical or managed care environment.
• Associate’s degree in Nursing.
• Active, unrestricted Texas Registered Nurse (RN) license.
• BLS for Healthcare Providers certification.
• Maintain a primary residence in Texas and the licenses and credentials required for the position throughout employment.
