ComForCare Home Health Care - Florida Tampa/NW Tampa is looking for a dedicated and compassionate RN Case Manager to join our growing team in Tampa/NW Tampa, FL. Requirements
- Active and unrestricted Registered Nurse (RN) license in the state of Florida
- Prior experience in home health, case management, or community health nursing preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills with patients and families
- Ability to work independently and manage a flexible schedule in the field
- Reliable transportation and valid driver's license required
- Proficiency with electronic health records (EHR) and clinical documentation
Responsibilities
- Conduct comprehensive in-home patient assessments to determine care needs and eligibility
- Develop and document individualized care plans based on assessment findings
- Evaluate patient health status, functional abilities, and home environment safety
- Collaborate with physicians, care teams, and family members to coordinate appropriate services
- Ensure accurate and timely clinical documentation in compliance with regulatory standards
- Monitor and reassess patient conditions, adjusting care plans as needed
- Serve as a patient advocate, connecting clients with community resources and support services
