As a Home Health & Hospice RN Case Manager, you will play a vital role in guiding patients and families through their healthcare journeys. From supporting recovery and independence through home health services to providing compassionate end-of-life care through hospice, you will combine clinical expertise with genuine compassion to make a meaningful impact every day. RN Case Managers are trusted advocates who coordinate care, perform comprehensive assessments, collaborate with interdisciplinary teams, and build lasting relationships with patients and their loved ones.
Requirements
- Current RN license
- Current CPR certification
- Strong assessment and clinical decision-making skills
- Experience coordinating patient care and services
- Excellent communication and relationship-building abilities
- A compassionate, professional, and patient-focused approach
- Commitment to providing high-quality home health and hospice care
