About The Position

Wilson Health is seeking an experienced Home Health & Hospice leader who combines strong operational and clinical expertise with genuine compassion for patients, families, and caregivers. The Director of Home Health & Hospice is responsible for the overall leadership, strategic direction, clinical quality, operational performance, and financial stewardship of Wilson Health's Home Health and Hospice programs. This leader will oversee day-to-day operations while ensuring exceptional patient outcomes, regulatory compliance, staff engagement, quality improvement, and organizational growth. The Director will also play an important role in supporting the department's continued evolution, including optimization of clinical workflows and technology platforms.

Requirements

  • Current Registered Nurse (RN) licensure- Ohio
  • Bachelor's Degree in Nursing
  • Minimum of five (5) years of healthcare leadership experience
  • Previous Home Health leadership experience
  • Previous Hospice leadership experience
  • Demonstrated experience leading clinical teams, managing operations, and ensuring regulatory compliance

Nice To Haves

  • Master's Degree in Nursing, Healthcare Administration, or related field
  • Experience leading both Home Health and Hospice services
  • Director-level leadership experience
  • Experience with Meditech and/or MatrixCare
  • Experience managing budgets, quality programs, and strategic initiatives

Responsibilities

  • Provide overall leadership and operational oversight of Home Health and Hospice services
  • Lead, coach, mentor, and develop clinical and support staff
  • Foster a positive, collaborative, and patient-centered culture
  • Ensure compliance with federal, state, accreditation, and regulatory requirements
  • Oversee staffing, scheduling, performance management, and employee development
  • Develop and maintain departmental policies and procedures
  • Monitor and improve quality outcomes, patient satisfaction, and performance metrics
  • Manage departmental budgets and financial performance
  • Lead quality improvement and process improvement initiatives
  • Support and collaborate with volunteer services and community partners
  • Partner with physicians, hospital leadership, patients, families, and community agencies to ensure exceptional care delivery
  • Maintain oversight of documentation, quality reporting, and clinical standards
