About The Position

Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. As a Nurse Case Manager, you guide patients and families through the care continuum by assessing needs, coordinating care, supporting discharge and transition planning, and managing ongoing outpatient care. Under the supervision of an RN or MD, you help ensure patients receive the appropriate level of care, provide education, coaching, and treatment-decision support, and collaborate with a Care Advocate and Social Worker. The position includes patient visits and certain hourly-paid non-visit activities. Visits and assignments vary based on patient and business needs. The role also includes assigned on-call responsibilities and services to patients and families.

Requirements

Current RN license specific to the state(s) assigned for work.
Valid driver's license, reliable transportation, and liability insurance.
Current CPR certification.
At least 1 year of clinical experience as a Registered Nurse (RN).
Pass a drug test before beginning employment.

Responsibilities

Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
Make referrals to other disciplines based on the patient's identified needs, or document the rationale for not making referrals.
Promote patient health and independence through teaching and appropriate rehabilitative measures, helping patients learn appropriate self-care techniques.
Supervise LPNs and HHAs.
Complete documentation timely and accurately.
Regularly communicate patient progress to the clinical manager and care team.
Plan and provide staff education.
Perform assigned on-call responsibilities and provide on-call services to patients and families.
Participate in clinical development and continuing education programs.
Perform other duties as assigned.
