About The Position

The Hospice RN Case Manager delivers hospice care to patients in their homes and communities across Mesa, Chandler, and Gilbert. The role guides families through care decisions and provides comfort, dignity, and compassionate support. This is a full-time position with a Monday-Friday schedule.

Requirements

• Currently licensed as a Registered Nurse (RN) in the State of Arizona.
• Minimum of 2 years of nursing experience.
• Valid driver’s license and reliable, insured vehicle.
• Strong clinical skills and a passion for patient-centered care.

Nice To Haves

• Hospice or home health experience preferred.
• Homecare Homebase (HCHB) experience preferred but not required.

Responsibilities

• Complete comprehensive assessments of patients and families to determine hospice needs.
• Develop individualized care plans and revise them as needed.
• Provide skilled nursing care, administer medications, and manage symptoms.
• Educate patients and families on care techniques, medications, and comfort measures.
• Maintain accurate clinical documentation and communicate changes to physicians.
• Participate in interdisciplinary team meetings and quality improvement initiatives.
• Provide emotional support and counseling to patients and families.
• Supervise hospice aides and delegate responsibilities appropriately.
