About The Position

Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The RN Case Manager coordinates compassionate, individualized hospice care by assessing patients, partnering with the interdisciplinary care team, and developing plans that support each patient's needs. The role includes on-call responsibilities and services as assigned. The position offers development opportunities and performance recognition. Salary ranges from $75,620 to $113,430 annually based on full-time employment. Pay is determined by factors including local labor markets, education, work experience, and certifications. Requirements

Current RN license specific to the state or states where assigned to work.
At least 1 year of nursing experience.
Current CPR certification.
Valid driver's license, reliable transportation, and liability insurance.
Must pass a drug test before beginning employment.

Responsibilities

Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
Makes referrals to other hospice care disciplines as needed or documents the rationale for not doing so.
Develops, prepares, and maintains individualized patient care progress records accurately, promptly, and according to care center policies.
Records pain and symptom management changes and outcomes as appropriate.
Teaches patients, families, and caregivers appropriate patient care techniques.
Regularly communicates with the clinical manager and care team.
Performs on-call responsibilities and provides on-call services to patients and families as assigned.
Performs other duties as assigned.
