About The Position

As an RN Case Manager, you’ll coordinate compassionate, individualized hospice care by assessing patients, partnering with the interdisciplinary care team, and developing plans that support each patient’s needs. The role includes patient and caregiver education, care documentation, symptom-management tracking, regular team communication, and on-call duties as assigned. Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The position offers a salary range of $91,540 to $137,310 annually based on full-time employment. Pay may vary according to factors including local labor markets, education, work experience, and certifications. Benefits and incentives are subject to eligibility requirements. Candidates must pass a drug test before beginning employment.

Requirements

Current RN license specific to the state(s) assigned for work.
At least 1 year of nursing experience.
Current CPR certification.
Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
Makes referrals to other hospice care disciplines as needed or documents the rationale for not doing so.
Develops, prepares, and maintains individualized patient care progress records accurately and promptly, in accordance with care center policies.
Records changes and outcomes in pain and symptom management as appropriate.
Teaches patients, families, and caregivers appropriate patient care techniques.
Regularly communicates with the clinical manager and care team.
Performs on-call responsibilities and provides on-call services to patients and families as assigned.
Performs other duties as assigned.
