About The Position

Care Initiatives Hospice is hiring a compassionate Registered Nurse (RN) Case Manager to provide comprehensive, personalized care to patients with life-limiting illnesses. The RN Case Manager meets patients and families in settings where they are most comfortable, including private homes, long-term care communities, assisted living residences, and hospitals. Care takes a holistic approach, addressing physical, emotional, and spiritual needs and working collaboratively with families and caregivers to honor patients’ needs and wishes with compassion, dignity, and respect.

Requirements

• Active Iowa RN license.
• Active CPR certification.
• Hospice or palliative care experience.
• Current driver's license and automobile liability insurance.
• Ability to travel between multiple locations within the region.
• Home access to the internet.
• Ability to obtain Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months of employment.

Nice To Haves

• Two years of verifiable experience working as a nurse (RN/LPN) is preferred.

Responsibilities

• Use clinical nursing knowledge, physical assessment, and procedural skills to deliver high-quality patient care in the patient’s place of residence.
• Develop and oversee the plan of care under the direction of the physician, ensuring appropriate interdisciplinary involvement.
• Perform initial and comprehensive patient assessments in accordance with policies and procedures.
• Conduct ongoing visits and interdisciplinary assessments of patients.
• Deliver quality care and complete clear, concise, and accurate clinical documentation.
• Provide patient, caregiver, and family counseling and education as indicated in the plan of care.
• Develop and continually update the plan of care to identify the care and services required to meet patient-specific needs.
