About The Position

Pediatric Nursing Specialists of Indiana (PNS), a Home Care Network (HCN) agency, is seeking a highly skilled, experienced, accountable, and self-motivated home health RN Case Manager. This role involves a mix of case management, recertifications, starts of care (SOC), resumptions of care (ROC), other assessments and services, occasional Skilled Nursing visits, as well as new hire orientations, trainings, in-services, and supervisory reviews. The position requires travel to multiple counties but offers a consistent schedule to fit personal/family commitments, with no overnight shifts or overnight travel. This is a PRN position with a long-term employment opportunity that can be built to fit your schedule, requiring travel up to 2 hours. PNS/HCN is a Medicare-certified home care and home health agency providing total patient & family support for infants, pediatrics, adults, and seniors. Requirements

  • Graduate of an accredited School of Nursing.
  • Minimum of one (1) year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Indiana.
  • Minimum of one (1) year of experience in a home health or acute care setting or equivalent experience. 1+ years of paid home health / in-home care experience is also required.
  • Minimum of one (1) year Experience in case management is required.
  • CPR and First Aide Certifications required (existing or can be obtained prior to hire date through our agency or accredited third parties).
  • Current Driver's License and reliable transportation.
  • At least 1 year of Case Management experience and home health agency (or equivalent) experience are required.
  • Must have experience with OASIS and NON-OASIS assessments.

Nice To Haves

  • 1 or more years of experience in home health care, wound care, lab draws, and OASIS assessments is a plus.
  • Experience and willingness to serve clients of all ages (infants, pediatrics, adults, seniors)
  • Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus.
  • 2+ years of both home health and acute care experience are preferred.

Responsibilities

  • RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.
  • RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • RN to Document/Communicate/Coordinate patient status/change in condition to care team.
  • RN to perform job duties while promoting personal safety and a safe environment for clients and coworkers.
  • Willing to travel surrounding counties.
