About The Position

Amedisys is hiring a full-time, day-shift Registered Nurse Case Manager for home health. The role serves patients throughout East Tucson, Northeast Tucson, Catalina Foothills, Rita Ranch, and Vail. The Case Manager assesses patients, coordinates care, supports patient health and independence through teaching and rehabilitative measures, and supervises LPNs and home health aides. The position also includes timely documentation, communication with the clinical manager and care team, staff education, assigned on-call responsibilities, and participation in clinical development and continuing education. The advertised compensation is $85,000–$97,000. Mileage is reimbursed at $0.53. Requirements

At least one year of clinical experience as a Registered Nurse; candidates with less than one year must be approved by the VP of Clinical.
Current RN license specific to the state or states where the employee is assigned to work.
Current CPR certification.
Valid driver's license, reliable transportation, and liability insurance.
Candidates with less than six months of clinical RN experience must participate in the RN Intern program.

Responsibilities

Perform patient assessments and collaborate with the care team to develop and implement plans of care.
Refer patients to other disciplines as indicated by identified needs, or document the rationale for not making a referral.
Promote patient health and independence through teaching and appropriate rehabilitative measures, including teaching self-care techniques.
Supervise LPNs and home health aides.
Complete documentation accurately and on time.
Regularly communicate patient progress to the clinical manager and care team.
Plan and provide staff education.
Perform assigned on-call responsibilities and provide on-call services to patients and families.
Participate in clinical development and continuing education programs.
Perform other duties as assigned.
